Labs

Creatinine (mg/dL)

Cr

Capped 1.0–4.0; dialysis sets creatinine to 4.0.

Bilirubin (mg/dL)
i
Bili

Minimum 1.0 for the formula.

INR
i
INR

Minimum 1.0 for the formula.

Sodium (mEq/L)
i
Na

Clamped 125–137 for MELD-Na.

Dialysis (≥2× in last week)?
i
Dialysis

Sets creatinine to 4.0.

How to use this calculator

  1. Enter creatinine, bilirubin, INR, and sodium.
  2. Mark dialysis if applicable.
  3. Read MELD and MELD-Na.
  4. Interpret only with a clinician — listing priority is not this page’s job.

Results explained

MELD combines creatinine, bilirubin, and INR with published coefficients and rounding. MELD-Na further adjusts for hyponatremia when MELD is above 11. Lab units, dialysis rules, and transplant policy evolve — this is arithmetic practice, not allocation advice.

Quick reference: MELD

MELD = round(10×(0.957 ln Cr + 0.378 ln bili + 1.120 ln INR + 0.643)).

How the estimate is built

Clamp labs; MELD as published; if MELD>11, MELD-Na = MELD + 1.32(137−Na) − 0.033×MELD×(137−Na).

Example scenario

Cr 1.2, bili 2.0, INR 1.5, Na 135 → MELD about 15, MELD-Na about 17.

FAQ

SI units?

This page expects mg/dL creatinine/bilirubin and mEq/L sodium.

Why floors at 1.0?

The original formula clamps low values to avoid extreme logs.

Is MELD-Na always used?

Policies vary; both scores are shown for learning.

Can I use this for prognosis alone?

No — clinicians use full context.
